What does a valid sitemap need?

Well-formed XML in the sitemaps.org namespace, absolute URLs, under 50,000 entries and 50 MB per file, and only canonical, indexable pages. Optional lastmod values should be accurate, because search engines may ignore dates they consider unreliable.

Why are my sitemap URLs not indexing?

A sitemap is a discovery hint, not a guarantee. Pages may be excluded for quality, duplication, noindex directives, robots.txt blocks, or weak internal linking. Fix those causes rather than resubmitting the same URLs.

Should I include every page in my sitemap?

Include canonical pages you want indexed and omit redirects, error pages, duplicates, and anything blocked by robots.txt. Large sites should split sitemaps by type and reference them from a sitemap index.
